As a patient or caregiver, you can use the MyHealth360 mobile app to access:
- Radiology reports
- Laboratory reports

How it works
During your clinic visit, we will collect your mobile number to send you an SMS notification once your results are available in the MyHealth360 app.
Medical Records Office (MRO)
Alternatively, you can visit our Medical Records Office for assistance. Before reaching out to the MRO, please have the following documents ready:
- An Application and Consent for Release of Health Records (PDF) from the patient to release the medical information.
- A copy of the patient’s and authorised requestor’s NRIC or passport for verification.
- (If the patient is below 21 years old) The application and consent of the patient’s parents or legal guardian (PDF).
- (If the patient is deceased or lacks mental capacity) The application and consent of the patient’s legal representative (PDF). Where relevant, a copy of the death certificate may be required.

Please allow a minimum of 5 working days for the processing of discharge summaries and Radiology/Laboratory results requests. Requests for other health information will be assessed on a case-by-case basis, and applicable administrative fees may be charged. You may receive your requested health records through email, mail or self-collection at our MRO.
